Consultative services
Most patients with SUD enter the medical system in settings unequipped to address their needs. Steadfast Health partners with hospitals, ERs, and other outpatient providers to provide same-day consultative services, ensuring that patients receive treatment when they need it most. This philosophy breaks down the siloes in the fragmented addiction system, reducing life-threatening relapses and increasing retention in longitudinal treatment.
Chronic disease recognition
We understand that addiction is a chronic, relapsing condition. Because we care for our patients across their lifetime, we ensure that if relapse occurs, patients can re-enter care with providers who know them well, either through ambulatory detox services or clinician-to-clinician coordination if higher levels of care are needed. Our approach minimizes interruption to patients’ lives and avoids the need for disruptive, costlier services by quickly re-establishing support and treatment.
Longitudinal care coordination
We accompany our patients for life. Our entire staff (clinicians, therapists, recovery coaches, and care coordinators) takes ownership of patient care plans, ensuring smooth transitions and uninterrupted treatment as patients transition across levels of care. We also recognize the many competing socioeconomic demands standing in the way of recovery. Our expert recovery coach team ensures that social determinants of health are addressed from day one as an integral component of our treatment plan.
